With the five year strategic plan (2019-2024) co-created with boys and young men of color, community partners and staff in 2018 it was decided to "transition" the leadership of Together for Brothers from the Co-Founder and Executive Director, Christopher Ramirez, to a new leader. In 2024 and 2025 this happened in a 12-month process when the Program Director, Baruch Campos, agreed to step into the Executive Director role.

The 12-month transition process began with co-creating a timeline with Kay Bounkeua and Ma Der Collaborations that included a $60,000 budget that was funded by Albuquerque Community Foundation, Energy Foundation, NewMexicoWomen.Org, Schmidt Family Foundation, TK Foundation and United Way of North Central New Mexico.

The timeline included collecting feedback with the Ford Foundation's Organizational Mapping Tool (OMT) with support from the TK Foundation and Engage Research & Design. The survey included more than 100 stakeholders in English and Spanish as well as a strategic planning retreat and focus groups with boys and young men of color as well as community partners to review the feedback and transition timeline. In addition individual and group coaching was planned with the outgoing and incoming Executive Directors. T4B also documented the process with a graphic facilitator, Taslim Von Hattum. And finally T4B culminated the transition with a T4B 10-Year Celebration and Fundraising Breakfast in August 2025.
