T4B Travel Release & Health Consent

We certify and agree that:

• I am the parent or legal guardian of "youth”.

• With my consent, the youth will travel with Together for Brothers (T4B) on the dates listed accompanied by T4B staff and adult representative who will be in charge of the youth person during this trip.

• I agree that Together for Brothers (T4B) has permission to care for or supervise the “youth” during this travel or the convening.

• I release Together for Brothers (T4B) and their trustees, officers and employees from, and agree to hold harmless them and from, any liability or claim in relation to the youth, including, but not limited to, personal injury, however caused, which may occur during or in relation to this trip or the convening.

• The “youth” and I agree that the youth will follow community agreements including:

• • Following all local, state and national laws

  • Not leaving lodging or other event locations by herself/himself/themself

  • Not consuming any alcohol or drugs

  • Remembering that he represents T4B and local community in all actions

• In the event my “youth”, a minor, becomes ill or sustains an injury while traveling, I give my permission to T4B to administer first aid to my “youth.” If I, (the parent, the legal guardian), cannot be contacted immediately in the event of an emergency, I authorize, representatives of T4B, to consent to emergency hospital care for my child.  I assume full responsibility for all related medical costs.

Autorización de viaje y consentimiento de salud T4B

Certificamos y aceptamos que:
• Soy el padre o tutor legal de un "joven".
• Con mi consentimiento, el joven viajará con Together for Brothers (T4B) en las fechas indicadas acompañado por el personal de T4B y un representante adulto que estará a cargo del joven durante este viaje.
• Acepto que Together for Brothers (T4B) tiene permiso para cuidar o supervisar a los “jóvenes” durante este viaje o la convocatoria.
• Libero a Together for Brothers (T4B) y a sus fideicomisarios, funcionarios y empleados, y acepto eximirlos de toda responsabilidad o reclamo en relación con el joven, incluidos, entre otros, lesiones personales, cualquiera que sea su causa, que puedan ocurrir durante o en relación con este viaje o la convocatoria.
• El “joven” y yo acordamos que el joven seguirá los acuerdos comunitarios que incluyen:
⁃ Cumplir con todas las leyes locales, estatales y nacionales.
⁃ No salir solo del alojamiento u otros lugares del evento
⁃ No consumir alcohol ni drogas.
⁃ Recordando que representa a T4B y a la comunidad local en todas las acciones.
• En caso de que mi “joven”, un menor de edad, se enferme o sufra una lesión mientras viaja, doy mi permiso a T4B para que le administre primeros auxilios a mi “joven”. Si no se puede contactar a mí (el padre, el tutor legal) de inmediato en caso de una emergencia, autorizo a los representantes de T4B a dar mi consentimiento para la atención hospitalaria de emergencia de mi hijo. Asumo total responsabilidad por todos los costos médicos relacionados.
